happened to me at least more than 100 times. I used to take a ss every time because I was dumb enough thinking I would get the mana back, but it's only 15 mana after all! But the pain is real...

Happens all the time to me. Sometimes I don't even get mana from the scrambler even though it promises mana :/

Not actually sure what causes this but I've seen it happen on other servers before - the chat event is still being fired and the message is being sent, so the message is being registered somewhere, it's just not picking it up for the unscramble.
Weird - annoying, but not too high priority to fix.
